By keeping track of your family&#8217;s medical history, your doctor may be able to detect diseases you&#8217;re likely to develop before they get the best of you.</p>
<p> Whom should you ask to find out about your family&#8217;s health/medical history? The best people to ask are your mother, father, siblings, aunts, uncles. If you do not know your biological parents or siblings, doctors suggest that you search for their death certificates, or talk with their physicians and access their hospital records.</p>
<p>          ]]&gt;</p>
<p> What information should you document?</p>
<p> Causes of death<br />
 Diseases and disorders such as visual impairment, glaucoma, hearing impairment, obesity, diabetes, high blood pressure, coronary heart disease, cancers, depression, muscular dystrophy, Tay-Sachs disease, hypothyroidism, Down syndrome, etc. </p>
<p> </p>
<p>Habits such as smoking, diet, alcohol or drug use, exercise, and fitness. </p>
<p>Where should you keep the information? Record your family&#8217;s health/medical history in a journal. And if you&#8217;d like, create a chart or family tree listing any health traits under each person&#8217;s name. Store the journal in a safe box with other invaluable records, such as birth certificates and home mortgages. </p>
<p>But remember to take the journal with you whenever you or your immediate family members are scheduled to see your family physician. Share all of your findings on your family&#8217;s medical history with your doctor and record any new information after each physical.</p>

			<content:encoded><![CDATA[<p>Sharing a doctor to increase productivity? Sharing a doctor&#8217;s appointment to bond with other patients suffering from the same chronic condition? It is the kind of thing that concierge doctors are concerned over. Imagine paying full price, or your full co-payment, and going to a shared doctor&#8217;s appointment with 30 other patients who might be experiencing the same chronic condition that you are. Does this sound like a good idea, or a recipe for disaster?</p>
<p> </p>
<p>&#8220;Shared medical appointments improve patient access, enhance patient and physician satisfaction, and increase practice productivity, all without adding more hours to a physician&#8217;s work week. There is even evidence that they promote better outcomes and lower overall costs of care.&#8221; That&#8217;s according to ManagedCareMag.com.</p>
<p> </p>
<p>Lets add some insight into the previous image; imagine paying full price for a doctor&#8217;s visit, visiting with that doctor in a room full of other patients, or ‘observers,&#8217; who are able to ‘sit-in&#8217; on your doctor&#8217;s appointment, share ideas, discuss symptoms, and listen to every word that you are telling your doctor. Not much room for privacy, huh?</p>
<p> </p>
<p>And when it comes to privacy, there are two different thoughts on the matter. One patient told NBC that his experience with the shared doctor&#8217;s appointment was not all it was cracked up to be; &#8220;One on one I can talk to the doctor and ask personal things, not that I can&#8217;t do that here but I don&#8217;t want to take up the time.&#8221;</p>
<p>          ]]&gt;</p>
<p> </p>
<p>And yet a physician told another media out let the exact opposite; &#8220;The biggest surprise was patient confidentiality,&#8221; says Rajan Bhandari, MD, chief of neurology at the Kaiser Permanente Santa Theresa Medical Center in San Jose. &#8220;They reveal more about themselves than I would ever have known about them otherwise. They seem to really blossom when they&#8217;re in a warm, empathic environment where they feel nurtured, supported, and not alone.&#8221;</p>
<p> </p>
<p>While the money spent is exactly the same, the confidentiality seems to be lacking, and the overall medical treatment might be deficient, physicians say the &#8220;real benefit is that instead of pretending that patients who have been living with chronic medical conditions don&#8217;t know anything about them, you actually involve them in the care-giving process.&#8221;</p>
<p> </p>
<p>According to ManagedCareMag.com, a two-year study funded by the Robert Wood Johnson Foundation showed that patients participating in the cooperative-clinic model stayed independent longer and were more satisfied with their physicians and with their understanding of their medical conditions. Physician satisfaction also increased, while hospitalization and ER use decreased by 12 and 18 percent, respectively. Cooperative-clinic participants were 2.5 times as likely to stay with their physician and with Kaiser.</p>
<p> </p>
<p>This method of medicine becomes not so much about the chronic condition itself, but about the person living with the chronic condition. This bonding between patients with like conditions and the ability to help one-another out in these shared doctor appointments seems to offer an &#8220;installation of hope.&#8221; In shared doctor appointments, patients no longer feel like they&#8217;re the only ones dealing with the chronic condition. They can see others living with the condition as well, whether in a greater way or a less fortunate way.</p>
<p> </p>
<p>Another aspect of shared doctor appointments is the time spent with the doctor, though it might be ‘shared&#8217; time. A general appointment with the family physician will run from between 8 to 10 minutes, while in a shared appointment that time is extended to 90 minutes, a benefit that makes patients feel as if their getting their money&#8217;s worth.</p>

<p>While it might be a little different, and may take some getting used to, it is creating a buzz in the medical community and it is getting people excited about more possibilities for healthcare. Shared doctor appointments are bringing more attention to the fact that patients are frustrated with the system, with the way they are treated in their 8 minute doctor appointments, and that they are looking for alternatives to general medicine.</p>

			<content:encoded><![CDATA[<p>While they are often the most visited physicians in the country, there is an increasing shortage in the number of family practice physicians. The trend began almost 4 years ago, when statisticians noticed that less and less graduating medical students were choosing primary care, or family practice, as their specialization. In a recent survey conducted by Merritt Hawkins &amp; Associates, for the Physicians&#8217; Foundation, it was found that about 150,000 (or half) of the country&#8217;s family practice physicians planned to quit practicing or see fewer patients over the next three years. With less family medicine physicians entering the workplace and more physicians leaving family practice, it is sure to create an abundance of available employment opportunities.</p>
<p>Physicians, who are in the phase of choosing a specialty, have been gravitating towards other, more focused, fields of medicine. With the ability to charge a premium and focus on one particular region of medicine, choosing a focused specialty can be alluring and lucrative. Having found their particular niche in medical school, or perhaps during internship, most prospective physicians are choosing to remain within that focus when seeking employment. Family practice treats a broad audience, and a broad knowledge is needed in order to properly treat these patients. While it may not be as enticing, it is often just as rewarding.</p>
<p>What seems to be shocking is the finding that family practice physicians are planning to leave their practices, or reduce their number of patients over the next three years. Many physicians cite frustrations with the current Medicare reimbursement structure. When Medicare does not reimburse the physician for the full cost of the services provided to the patient, the physician is forced to bill the remaining balance to the patient. It is often that these patients are unable to pay the remaining balance because they are elderly citizens living on a fixed income, and many of them do not carry supplemental health coverage. Physicians who are self employed, working as an independent practice, cite that they have experienced a dramatic increase in the required non-clinical paperwork required by Medicare.</p>
<p>We all know how frustrating paperwork can be! With all the complaints filed it&#8217;s important to consider that although family practice physicians may be feeling it more strongly, these are concerns that have been voiced by physicians throughout the industry.</p>
<p>With the shortage of primary care physicians comes great hope for those seeking employment opportunities. There is growing availability in almost every area of the country, but specifically rural areas. With patients in great need of finding an established family practice, there are also growing opportunities for physicians looking to start their own practices. Small groups of physicians are opening family practices in farming communities and are finding it difficult to keep up with the amount of patients wishing to be seen. They are able to work together and spread the responsibilities of patient care, hospital rounds and call duties among the participating physicians. Often, they are successful enough to offer salaries and benefit structures that are competitive with urban markets. Additionally, these rural family practices serve the community, not only by providing much needed health care, but also by providing employment for support personnel.</p>
